Emergency Care Card Information Form (Required for all students)
Parents must review emergency care information on file and update the form (if needed). All parents need a SIS ParentVue account and can update the Emergency Care Form via weCare on the FCPS links button. Click this link to access the weCare@School information page.
For more information about obtaining a SIS ParentVue account, click this link to access the FCPS SIS Parent Account information page. If you update and submit Emergency Care through weCare, please print a copy to bring to school by Sept. 28. (2 pages).
Digital Resources @ MVHS (Required for all students)
In an ongoing effort to protect student data, FCPS is required to secure parental permission for student use of software/applications for which FCPS does not have a confidentiality agreement. An online process has been developed to collect this information digitally and provide school-based reporting. This process has been designed to meet the legal requirements for authenticating parent identities under FERPA.Parents, please follow the below link to provide permission for your student to access supplemental tech tools used at Mount Vernon High School. These tools are critical to creating an effective, engaging virtual classroom experience.
Health Information (Required for all students)
Complete this form annually to inform us about your student’s health condition that affects his or her school day.
Students' Rights and Responsibilities (SR&R Form) - (Required for all students)
Parents and guardians, please review the Student Rights and Responsibilities and related resources at by accessing the following SR&R link from the main FCPS website: www.fcps.edu/srr.

After reviewing the information, fill out the SR&R Parent Acknowledgement Form using this electronic acknowledgement page. The form must be filled out one time per student.
Alternatively, you can sign a printed copy of the SR&R signature page acknowledging you have received the SR&R information and return it to the school by October 29th, 2021.
Military Connected Students (Required for all students)
In accordance with the Code of Virginia (§22.1-287.04), local school divisions are required to identify students who have a parent in the United States uniformed services. Completing this form allows Virginia localities to maintain reliable and accurate data for potential grant funding and to receive services to meet the needs of uniformed services- connected students.

Family Life Education Opt-Out Request Form
Fairfax County Public Schools (FCPS) provides a comprehensive, sequential Family Life Education (FLE) program for kindergarten through grade twelve. The FLE program is designed to provide students with the knowledge, attitudes, and skills to make healthy, responsible, respectful, and life-enhancing decisions related to human growth and development, relationships, and emotional and social health. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.

FLE Opt-Out - To opt your child out of all of some of the Family Life Education lessons.
Clinic Health Forms (if applicable to your child)
Medication Authorization Form - Request personnel to administer medication to your child during the school day.
Student Use of Inhalers Form - Authorize your child to use an inhaler in school as prescribed.
Student Use of Epi-Pen Form - Authorize FCPS personnel to administer epinephrine injection(s) as directed by a physician.
Parent information regarding Student Opt-Out Rights and Choices (if applicable to your child)
Wellness Survey Opt-Out Form - 9th and 11th grades only; this opt out is for parents who do not wish for their child to participate in the SOS program.
